Reissue of certificates

The QSA provides certified copies of certificates and other educational results held by us. To protect people's privacy, copies can only be provided to the person named on the certificate.

Who to contact

Educational records are held by various agencies. The table below lists the organisations to contact to obtain replacement copies.

How to apply to the QSA

  1. Supply proof of identity and proof of name change (if applicable). A certified copy of one of the following is accepted:
    • Australian passport
    • birth certificate or extract of a birth certificate
    • Card 18+
    • driver licence or learner licence
    • immigration papers
    • naturalisation or citizenship certificate
    • student ID (with photograph).
    Documents must be current, and copies must be certified by a Justice of the Peace or a Commissioner for Declarations.
  2. Complete the following application form and post it along with your proof of identity and payment to the Queensland Studies Authority, PO Box 307, Spring Hill, Qld 4004.
  1. Costs are $22.50 for each copy. An additional charge of $5.40 applies for certificates posted to overseas destinations. Payment can be made by credit card, cheque or money order (made out to the Queensland Studies Authority). Overseas payments must be in Australian dollars.

Do not send either original documents or cash through the post. The QSA may charge to return original documents to you.

How long will it take to process my request?

Certificates will be available for posting or collecting within 3 working days following receipt of your correctly completed application.
